RESUMO

A 19-year-old patient after previous wedge resection of the right upper pulmonary lobe a year ago urgently admitted with recurrent right-sided spontaneous pneumothorax. According to standard management of spontaneous pneumothorax, we performed diagnostic thoracoscopy and drainage of the right pleural cavity with regular X-ray examinations. However, these measures were ineffective. The patient was scheduled for surgery, and we intraoperatively observed an unusual cause of pneumothorax. Thus, we present spontaneous pneumothorax following right upper lobe pulmonary sequestration. The uniqueness of this case is associated with unusual manifestation and non-standard localization of rare lesion. A few cases of pneumothorax in similar patients are described in the world literature. The key limiting factor in diagnosis of such defects (identification of aberrant vessel supplying abnormal lung parenchyma) is the lack of routine CT angiography in patients diagnosed with pneumothorax. That is why CT changes were interpreted as postoperative ones, and the true cause was established only during redo surgery. A thorough inspection of the pleural cavity and alertness regarding unusual appearance of the right upper pulmonary lobe made it possible to suggest a non-standard diagnosis, avoid complications (bleeding from afferent vessel) and perform adequate lung resection.

Spruce branch is a rare radiolucent bronchial foreign body. Despite modern imaging tests and endoscopic examination, this foreign body is often detected only intraoperatively. This study enrolled 4 patients with spruce branch aspiration. In the 4th case, spruce branch was removed during rigid bronchoscopy that was associated with «lodging¼ type of foreign body. In two cases, spruce branch migrated to peripheral bronchial segments («extrusive¼ type) that required surgical treatment (thoracoscopy with resection of the right basal pyramid segments and wedge resection of the right lower lobe). The 3rd case was the most interesting. Initially, the foreign body was «underlying¼, but it migrated after partial endoscopic removal that finally required right-sided lower lobectomy. This report describes the peculiarities of clinical course and management of patients with a rare type of radiolucent bronchial foreign body - spruce branch.

RESUMO

A study is presented on the effectiveness and safety of various anticoagulants used in patients of an older age group with acute coronary syndrome during percutaneous coronary interventions. Bivalirudin was shown to be highly effective in comparison with unfractionated heparin and monafram in relation to the amount of bleeding that occurs in the postoperative period and adverse cardiovascular complications.

RESUMO

Mechanical bowel preparation used to be a standard procedure for a long time. Nowadays routine use of MBP seems to be debatable thus alternative approaches, e.g. avoiding any bowel preparation completely or using of MBP with oral antibiotics are considered. Data on performing different kinds of bowel preparation is reviewed in this article.

RESUMO

Anastomotic leakage after surgery for colorectal cancer is a widely known factor aggravating immediate outcomes. At the same time, deterioration of oncological results is under much less attention. Long-term consequences of anastomotic leakage and possible mechanism of negative effect of this complication on long-term results are reviewed in the article.

RESUMO

Traditional methods for deriving computationally-generated collision cross sections for comparisons with ion mobility-mass spectrometry data require 3-dimensional energy-minimized structures and are often time consuming, preventing high throughput implementation. Here, we introduce a method to predict ion mobility collision cross sections of lipids and peptide analogs important in prebiotic chemistry and other fields. Using less than 100 2-D molecular descriptors this approach resulted in prediction errors of less than 2%.

RESUMO

Development of cancer surgery in recent decades occurs on a background of continuing scientific and technical progress. New technologies after the completion of clinical trials maximally quickly are included in the routine practice of specialized medical centers. At present an escalation of indications for extensive advanced and combined operations in locally advanced and even metastatic tumors goes in parallel with the introduction of minimally invasive interventions, search categories of patients, for whom the radicalism of treatment can be achieved without significant surgical aggression. The study of modern trends of this process will allow seeing the promising areas of scientific research, assuming the image of the future of surgery for cancer.

RESUMO

In the review published data on what modern antithrombotic drugs are used to support percutaneous coronary intervention in the treatment of acute coronary syndrome with ST segment elevation on the electrocardiogram in patients of older age groups. It was shown the effectiveness and safety of new pharmacological combinations using both anticoagulants and antiplatelet agents as in patients with acute coronary syndrome in elderly. It was highlighted also the need to find new combinations of anticoagulants and antiplatelet agents, to improve the prognosis in elderly persons subjected to percutaneous coronary intervention in patients with ACS.

RESUMO

The main treatment option for rectal cancer is surgery, which "gold standard" is the total mesorectumectomy. There are presented literature review and the results of own research devoted to comparative analysis of outcomes of laparoscopic and open total mesorectumectomy. Current data and own experience show the oncological adequacy and safety of laparoscopic approach however the controversy of some results reveal the necessity of further investigation.

RESUMO

Thoracic hematogenic endometriosis is a rare pathology. A clinical course hasn't pathognomic symptoms, because of it, the diagnosis is established due to histological study. The article presented two cases of female patients, who were suffering from thoracic endometriosis. They were hospitalized to the department of thoracic surgery of Municipal multifield hospital No 2 in Saint-Petersburg. The first patient had a posterior mediastinum tumor with asymptomatic disease course. The second patient was with recurrent catamenial pneumothorax.

The results are presented that describe the experimental study of colonic hemomicrocirculatory bed (HMCB) in intact rats, in animals 6 days after large bowel obstruction development that was modeled using the device created by the authors, and 1-15 days after its resolution. The most significant changes were observed in the capillaries and HMCB venular portion--capillary diameter was increased by 60-100%, while that of venules--by 73% as compared to those in control group. The degree of changes depended on the distance from the extraorgan bowel compression site. The dynamics of the normalization of morphometric parameters after colon obstruction resolution was demonstrated. It was found that the average arteriolar and venular diameters reached the initial level by day 5, while that of the capillaries by day 7 after colon obstruction resolution.

RESUMO

The purpose of this study was to assess the experience with long-term epidural space administration of 1% lidocaine in combination with general anesthesia. Anesthesia based on the long-term epidural lidocaine administration in combination with general anesthesia versus total intravenous anesthesia based in ketamine and ataralgesia was analyzed. The anesthesia based on the long-term epidural lidocaine administration in combination with general anesthesia was shown to provide steady-state hemodynamic parameters and an adequate antinociceptive protection and to contribute to the early activation of patients.

A comparative study of the thermostability of NAD+-dependent formate dehydrogenases (FDHs; EC 1.2.1.2) from both methylotrophic bacteria Pseudomonas sp. 101 and Moraxella sp. Cl, the methane-utilizing yeast Candida boidinii, and plants Arabidopsis thaliana and Glycine max (soybean) was performed. All the enzymes studied were produced by expression in E. coli cells. The enzymes were irreversibly inactivated in one stage according to first-order reaction kinetics. The FDH from Pseudomonas sp. 101 appeared as the most thermostable enzyme; its counterpart from G. max exhibited the lowest stability. The enzymes from Moraxella sp. Cl, C. boidinii, and A. thaliana showed similar thermostability profiles. The temperature dependence of the inactivation rate constant of A. thaliana FDH was studied. The data of differential scanning calorimetry was complied with the experimental results on the inactivation kinetics of these enzymes. Values of the melting heat were determined for all the enzymes studied.

RESUMO

The authors defined a radioactive waste neutralization and disposal site as a complex of industrial chambers with used territory of ground with burial places, buffer sanitary protection zone and observation zone. This area is a complicated geotechnical zone where natural communities develop under chronic exposure to natural and artificial radionuclides coming from both global and technogenic sources. Using geoinformative technologies of radioecologic safety, the authors evaluated protective functions of buffer zone, its biopotential and stability to specific type of nature management. The studies helped to identify sorption and migration capacity of the territory.

Anticarcinogenic action of dietary supplement Pheocarpin and its active component Natural Coniferous Complex (NCC), in particular, has been studied. Pheocarpin and NCC efficiently inhibited tumorigenesis in the mammary gland, large bowel, skin, cervix uteri and lungs. Pheocarpin offers considerable advantage as a means of reducing the risk of malignant disease.

In spite of achievements of modern pharmacology, duodenal ulcer (DU) incidence and, what is more, the frequency of complications, are still high. The presented study was carried out in order to evaluate somatic, personal, and psychological peculiarities of patients receiving different types of treatment for duodenal ulcers complicated by hemorrhage or perforation. The patients whose treatment for perforation or hemorrhage was not radical, demonstrated high relapse rate within 4 to 7 years after hospitalization (76% and 50%, respectively). Study of coping revealed high rate of "optimization " mechanism in the emotional sphere of the patients in both groups (56% in the group with perforation, 48%--in the group with hemorrhage). The dominance of this coping mechanism was associated with frequent refusals to receive repeated courses of antiulcer therapy. The study found significant difference in the cognitive and behavioral spheres of coping between the groups. So, "active avoidance" and "retreat" mechanisms predominated in the behavior of hemorrhage patients, which was accompanied by significant reduction in a range of life quality parameters. The obtained data correspond to the results of study of "internal picture" of the disease. Anosognostic attitude to the disease was predominant in perforation group (43.5%), while ergopathic attitude was typical of hemorrhage patients (28.6%). The results suggest that in order to increase the life quality and compliance of patients with DU, medical and rehabilitation measures should be individualized and optimized in accordance with personal and psychological peculiarities of these patients.

RESUMO

A new method for evaluating disorders in the sense of color has been developed and offered for practice. A total of 340 patients with diseases of the retina and optic nerve and 80 normal subjects were examined. The results indicate that different conditions of examination, selection of combination by the color, brightness, and deepness of the stimulus and background allow detection of decreased sensitivity of one, two, or three color perceptors (disorders in sense of color--color blindness) and permit a quantitative evaluation of functional disorders in the color channels of visual system. The new method can be used in the diagnosis of the initial pathological processes of different origin, involving the cone system at any level of visual route and associated with acquired disorders in color vision.

RESUMO

Surgical outcomes in 603 children with otitis media purulenta chronica (OMPC) have shown that early microsurgery (endaural tympano-, attico-, aditotomy) improved or reestablished normal anatomy and physiology of the middle ear in 85% of the patients. One-stage cleansing or plastic surgery is preferable. In operations on mastoid process in the absence of complications, prevention of reoperation seems most effective by autotissue plastic reconstruction of the mastoid bone wound in the end of the operation.

The inhibitory effects of the newly-developed forms of beta-carotene--water-soluble and liposomal--have been studied in rats and mice bearing tumors induced in 4 models of carcinogenesis. Mammary tumors were induced by single injections of 1 mg methylnitrosourea into each gland. Esophageal tumors were induced in male rats by intragastric administration of 3 mg/kg methylbenzylnitrosoamine, thrice a week for 4 weeks. Tumors of the vagina and cervix uteri were induced by intravaginal painting with 25 mkg dimethylbenz(a)anthracene, twice a week for 6 weeks. Tumors of the lung were induced in female mice by intraperitoneal injection of 1 g/kg urethane, once a week for 6 weeks. With models I-III, animals received water-soluble beta-carotene (Aquiton-10) with drinking water (200 mg/I), on completion of carcinogen treatment and for another 9-12 months until the end of experiment. Urethane-treated mice received liposomal beta-carotene with drinking water (60 mg/l) 10 days before the beginning of carcinogen treatment and for another 6 months until the end experiment. Water-soluble beta-carotene failed to influence the carcinogenesis in the mammary gland and esophagus in rats; however, it significantly inhibited carcinoma development in murine vagina and cervix uteri (47.0%). Liposomal beta-carotene significantly inhibited lung adenomas (46.4%) and mammary carcinomas (55.6%) in urethane-treated mice.
